Important Update for Canadian Medical School Graduates
Graduates of Canadian medical schools on or after July 1, 2025 are considered IMGs for the purpose of entering U.S. graduate medical education. These graduates generally establish eligibility for U.S. graduate medical education through ECFMG Certification.
Individuals who graduated from a Canadian medical school before July 1, 2025 are not considered IMGs and are not eligible for ECFMG Certification.

Step-by-Step ECFMG Certification Process in 2026
The ECFMG Certification process involves several connected stages. The overall timeline varies according to account establishment, identity verification, medical credential verification, examination preparation, medical school response times, and applicable Pathways requirements.
Step 1: Create a MyIntealth Account and Complete Identity Verification
The ECFMG Certification journey begins by establishing a MyIntealth Applicant Portal account.
Applicants must provide the required personal and medical education information, submit a digital photograph, upload a copy of their current, unexpired passport, and complete the applicable identity verification requirements.
After the initial account establishment request is processed, applicants complete the Intealth Identification Form (IIF) and participate in an online notarization session through NotaryCam.
The identity verification process includes a live online session with a licensed U.S. notary. Applicants have up to six months from the creation of their personalised IIF to complete the online notarization process.
After the identity verification process is completed and accepted, the applicant receives a unique MyIntealth Identification Number. This number is used to identify and track ECFMG-related services and applications.
For requests submitted on or after April 1, 2026, the MyIntealth Applicant Portal account establishment fee is US$110. This fee includes the US$50 NotaryCam fee for online notarization.
Step 2: Submit the Application for ECFMG Certification
After establishing a MyIntealth account and completing the required identity verification process, an applicant may submit the formal Application for ECFMG Certification.
The application confirms the applicant’s intention to pursue ECFMG Certification and collects the required personal and medical education information.
Applicants must provide information about their medical education credentials, including the applicable final medical diploma and final medical school transcript. The information and documents submitted must meet the relevant ECFMG credential requirements.
Applicants who have not yet graduated may be able to begin the certification process using their expected graduation information. However, the applicable final medical education credentials must be submitted and verified before ECFMG Certification can be completed.
For applications submitted on or after April 1, 2026, the Application for ECFMG Certification fee is US$580.
Step 3: Complete Primary-Source Credential Verification
ECFMG does not rely only on documents uploaded by an applicant. Required medical education credentials are subject to primary-source verification with the issuing medical school or other appropriate institution.
The verification process may include:
- Review of the submitted medical credentials

For requests submitted on or after April 1, 2026, the credential verification fee is US$220, including US$110 for the final medical diploma and US$110 for the final medical school transcript.
If an acceptable English translation is required for an applicable medical school transcript, an additional translation charge may apply. The listed translation fee is US$52 per page in applicable cases.
Applicants should also check whether their medical school or issuing institution charges a separate fee for responding to a credential verification request.
Step 4: Register for and Pass USMLE Step 1 and Step 2 CK
ECFMG Certification requires applicants to pass both USMLE Step 1 and USMLE Step 2 Clinical Knowledge (CK).
Following the USMLE service transition on January 12, 2026, USMLE Step examination services for IMGs are managed through the FSMB USMLE portal. ECFMG continues to manage ECFMG Certification and determine whether an IMG meets the applicable ECFMG eligibility requirements.
The general process includes:
- Establish eligibility for applicable ECFMG services through MyIntealth
- Access the FSMB USMLE portal
- Submit an application for USMLE Step 1 or Step 2 CK
- Complete the required examination registration information
- Pay the applicable examination and testing-region fees
- Receive the examination scheduling permit
- Use the information in the permit to schedule the examination at an approved testing centre
- FSMB confirms the applicant’s ECFMG eligibility as part of the USMLE registration process.
Applicants do not need to complete ECFMG Certification before taking Step 1 or Step 2 CK. However, they must meet the applicable ECFMG eligibility requirements before their USMLE examination registration can proceed.
The scheduling permit contains important information needed to arrange the examination appointment, including the relevant eligibility information and examination identification details.
Applicants should schedule their examinations as early as possible after receiving the required permit because appointment availability may vary by testing location and examination period.
Step 5: Complete an Approved Pathway and OET Medicine
Applicants using the 2026 ECFMG Pathways must meet the applicable clinical and communication skills requirements through an approved Pathway.
The 2026 Pathways included six available options. The appropriate Pathway depended on factors such as the applicant’s medical education, medical licensing status, clinical skills assessment, and other eligibility criteria.
All applicants to the 2026 Pathways, including applicants seeking Pathways revalidation, were required to achieve a satisfactory score on OET Medicine, regardless of citizenship or native language.

The clinical encounters had to take place in a formal outpatient clinical setting involving registered outpatients. Encounters involving patients receiving care in an emergency department, a separate dedicated urgent-care facility, or an inpatient hospital setting were not acceptable.
The 2026 Pathways application fee was US$925. For 2027, the Pathways application fee is US$945.
Step 6: Receive the ECFMG Certificate
ECFMG issues the ECFMG Certificate after an applicant has completed and met all applicable certification requirements. Depending on the applicant’s circumstances and certification pathway, these requirements may include:
· Completion of the required medical education credential process
· Primary-source verification of applicable medical credentials
· Passing USMLE Step 1
· Passing USMLE Step 2 CK
· Completion of the applicable Pathways requirements
· Meeting the required OET Medicine standards
Once all applicable requirements have been completed and verified, ECFMG may issue the ECFMG Certificate.
The certificate supports an IMG’s progression toward ACGME-accredited residency or fellowship training, eligibility for USMLE Step 3 when other applicable requirements are met, and future medical licensing.
However, ECFMG Certification does not guarantee residency placement or provide an unrestricted licence to practise medicine. Applicants must still meet residency program requirements, complete the required postgraduate medical training, and satisfy the applicable examination and state medical licensing standards.
ECFMG Certificate Validity
The validity of an ECFMG Certificate may depend on how the applicant satisfied the clinical and communication skills requirements for ECFMG Certification.
Applicants who met these requirements through a valid passing performance on the former USMLE Step 2 Clinical Skills (CS) examination receive an ECFMG Certificate that is not subject to expiration. These certificates are valid indefinitely.
Applicants who meet the clinical and communication skills requirements through the ECFMG Pathways receive certificates that are generally subject to expiration. The expiration date is based on the Pathways application season in which the applicant’s Pathway was accepted.

· Holds an unrestricted licence to practise medicine in a U.S. state, U.S. territory, or the District of Columbia.
A valid, unexpired ECFMG Certificate is generally required for entry into ACGME-accredited U.S. graduate medical education and for IMGs seeking eligibility to apply for and take USMLE Step 3, subject to all other applicable requirements.
Applicants should review the current ECFMG requirements because Pathways expiration, certificate validity, revalidation, and permanent validation rules may change and may depend on the applicant’s individual circumstances.
